Pre-Conference Questionnaire

Dear Parents,

Conference time is approaching. Please take a moment to respond below. The questionnaire will help me plan for your child’s conference so that we may discuss your child’s progress and answer questions that you may have regarding your child’s Kindergarten experience and learning. Please complete and send this form back to school with your child by Wednesday, October 1, 2008. Next week I will contact families to schedule a day and time.
